We do require a current picture ID or driver's license from the state of Louisiana -- this establishes residency. If a client arrives without a food stamp card, we require a notarized statement of income.

We require that each client establish an "emergency medication fund" of $20.

If a client does not have an active food stamp card we ask that family contribute $200 to assist with the first month of groceries
Medicaid or Medicare card or denial letter or proof of pending application
Or a clearance letter at Medicaid office on Louisville behind Grandy's

if coming from OCS, a clear letter of boundaries
Name, location, and phone number related to case worker (OCS, GoCare...)

If you are planning to bring a child on site - need social security card and birth certificates of all children. You must also agree to have child evaluated by outside agencies if recommended.
